1) Name of child.
2) Date and year of birth.
3) Hour of birth - indicate AM or PM.
4) Birth weight.
5) Place of birth - city and state.
6) Name and address to which the plate should be sent.
7) Size of Plate (6", 8" or 10"); Blue Tile or Color Tile

Wedding Plates/Tiles
1) Name of Groom
2) Name of Bride
3) Date of Wedding.
4) Place of wedding - city and state. 
5) Size of Plate or 10" plate or 6" Blue Tile

Anniversary Plate

Plates and tiles are individually hand painted and shipped from the Netherlands, so please expect between 4 and 6 weeks delivery time.
